About Proof of Immunity
Keio University is taking measures to prevent the spread of infectious diseases across its entire community,
including:- Faculty and staff members scheduled for work at Keio University Hospital or on Shinanomachi Campus due to employment, transfer, renewed contract work, etc.
- Faculty and staff members who are scheduled to return to Keio University Hospital following a temporary transfer
- New and continuing students who are scheduled to regularly visit Shinanomachi Campus
- Individuals who plan to study or travel abroad
Making sure students and faculty members are immunized against infectious diseases is an important precautionary measure to not only protect them as individuals but also to prevent them from transmitting diseases to those around them. Proof of immunization is also often required for traveling abroad.
Most students, faculty, and staff members who visit Shinanomachi Campus and the Keio University Hospital soon encounter training and work situations where they are in contact with patients and other students, faculty, and staff members. Therefore, we ask that you confirm your record of immunization against infectious diseases (e.g., MMRV*, hepatitis B) prior to enrollment or employment and visit a medical institution as soon as possible to receive any necessary tests or vaccines (we do not designate specific medical institutions for these services).
*MMRV: Measles, Mumps, Rubella, Varicella
Keio University Hospital Standard Work Protocol
1. MMRV disease
Applicant must meet one of the following conditions ①-③ for each MMRV disease:
②Proof of a past positive antibody test result (any year) and proof of a subsequent vaccination
③Proof of a past strongly positive antibody test result (any year)
・Measles: IgG-antibody titer determined by enzyme-immunoassay (EIA) or neutralizing antibody titer (NT)
・Mumps: IgG-antibody titer determined by enzyme-immunoassay (EIA)
・Rubella: IgG-antibody titer determined by enzyme-immunoassay (EIA) or hemagglutination inhibition (HI) antibody titer
・Varicella: IgG-antibody titer determined by enzyme-immunoassay (EIA) or immune adherence hemagglutination (IAHA) titer
2.Hepatitis B
Only required for those engaged in work or research that may involve exposure to bodily fluids, such as the handling of instruments or medical waste contaminated with body fluids, treatments involving contact with body fluids, blood sampling, testing, or specimen handling.Applicant must meet one of the following conditions:

If you are unable to complete all your vaccinations before you begin regular visits to Shinanomachi Campus, or if you are already working at Shinanomachi Campus, you can receive vaccinations at the below outpatient clinics at your own expense.

Immunization Record (as of April 24, 2023)
If you have received instructions from Keio University, Keio University Hospital, or another administrative office asking you to do so, please visit a medical institution (at your own expense) and have them complete an Immunization Record form on your behalf. Submit a PDF copy of this record to the Keio University Health Center within one month of starting work or study. (Please refer to the documents provided by your department for relevant links.) Failure to submit may result in restrictions on practical training, extracurricular activities, and employment.
Please note that the Keio University Health Center does not respond to inquiries regarding the loss of original documents. The Immunization Record is a useful document that can serve as proof of immunity for future travel, job changes, or academic enrollment. Please keep the original with you along with your other important medical documents, such as your proof of vaccination or your Maternal and Child Health Handbook.
Individuals already working at the Shinanomachi Campus who are eligible to visit the Keio University Health Center Outpatient Vaccine Clinic can have their Immunization Record prepared there at their own expense.
