Report when infected with an infectious disease

Students, Faculty & Staff from Outside of Shinanomachi Campus

If you become infected with an infectious disease designated by the School Health and Safety Act (hereinafter "infectious disease"), please report it using the form below. When returning to campus, follow the instructions in "Permission of Return to Campus" and contact the Health Center to schedule an interview to determine whether or not you will be permitted to return to campus.

Report Form(campuses other than Shinanomachi)

  • Students at the Faculty of Nursing and Medical Care and the Graduate School of Health Management should notify their instructors as soon as possible.
  • Students at Shiba-Kyoritsu Campus should follow any instructions given by their faculty.
  • Students at affiliated schools should notify the Health Center or health office at their school as soon as possible.

Students, Faculty & Staff at Shinanomachi Campus

As a medical institution, Shinanomachi Campus requires strict infection control measures. Be sure to follow the instructions of the hospital's Center for Infectious Disease and Infection Control and the Shinanomachi Branch of the Health Center.

Report Form (Shinanomachi Campus) (Japanese)

    *This applies to:
  • Members of the Keio Student Health Care Mutual Aid Association (Gakuseikenpo) at Shinanomachi Campus
  • Part-time instructors (fellowship training [diagnostic support] / education [丙] / child-rearing support / bedside learning [丙] / research scholarship)
  • Second- to sixth-year students at the School of Medicine
  • Students at the Graduate School of Medicine
  • Third-year students at the Faculty of Nursing and Medical Care

Reporting infections among group members, including at student dormitories, student clubs, or teams/clubs in the Athletic Association

If two or more persons are suffering from an infection at around the same time (fever of 37.5 degrees Celsius or higher; within 2 to 3 days in the case of influenza or COVID-19), please report this to the Health Center and take the appropriate measures as instructed in the PDF file below.

Report after infectious disease is resolved

●Other than influenza or COVID-19

If you have any of the above infectious diseases, according to the School Health and Safety Act , you should be excluded from attending school to prevent a spread of pathogens. Please ask a doctor to fill out a medical certificate that indicates the day you are permitted to return to campus, or the Permission of Return to Campus (a downloadable file from the Health Center website), and submit it to the Health Center.


If you have tested positive for influenza or COVID-19, please bring one of the following as proof of illness when obtaining permission to visit campus:

●If you have tested positive for influenza:

・A certificate of recovery or diagnosis issued by the health care center you visited
・A prescription instruction sheet or medicine bag with the date and your name written on it.
・If you have tested positive using a self-administered rapid antigen test kit, please provide a photo that shows the test kit's product name, manufacturer, and distributor, as well as the test results (control and test lines).(Print out and bring it with you.)

●If you have tested positive for COVID-19:

・A certificate of recovery or diagnosis issued by the health care center you visited
・If you have tested positive using a self-administered rapid antigen test kit, please provide a photo that shows the test kit's product name, manufacturer, and distributor, as well as the test results (control and test lines).(Print out and bring it with you.)
・If you have tested positive at a medical institution or testing facility, provide a paper copy or photo of your test result notification or a screenshot of your test result that includes your name, testing method, test result, and the name of the health care center or testing facility.(Print out and bring it with you.)
